The survey instruments that were used to collect data for the CASI study are available for your review.   Each instrument is accompanied by a description of its development and use.

Instruments are currently available for the following surveys:

  • Hospital Care Survey
  • Internal Medicine Physicians Survey (Versions A &B)
  • Family Medicine Physicians Survey (Versions A & B)
  • Pediatricians Survey (Versions A & B)
  • Chicago Community Asthma Survey (CCAS)

The survey instruments are currently in Adobe's PDF format. In order to read these, you must have the Adobe Acrobat plug-in.
